INFORMATION CLAUSE

In accordance with art. 13, it. 1 and it. 2 of the regulation of the European Parliament and the Council (EU) 2016/679 of 27th of April, 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data and repealing the Directive 95/46/EC (General Data Protection Regulation) (O.J. EU.L of 2016, No. 119 p.1) I hereby inform that:

1) the administrator of Your personal data is Anna Orska-Oleszczyk, conducting business activity under the name: ORSKA sp. z o.o. with the business site located in Poznań (60-406), 330 Dabrowskiego Street, Register of Companies conducted by the Mayor of Poznań No. 66615/2009, Tax Identification Number (NIP) 7812028156, Company Registration Number (REGON) 520499764, (hereinafter referred to as : ORSKA);

2) Your personal data will be processed for:

1. a) concluding and performing the sales agreement of the products by ORSKA, You being the party, on the basis of art. 6, it. 1 point b) of the above regulation;
2. b) preparing replies to the claims reported by You for the guarantee of quality or warranty for physical defects of the products by ORSKA, on the basis of art. 6, it. 1 point b) and c) of the above regulation;
3. c) marketing purposes, including participation in promotion campaigns concerning the products by ORSKA, after expressing separate consent, on the basis of art. 6, it. 1 point a) of the above regulation;
4. d) obtaining commercial information from ORSKA company including by e-mail on the basis of art. 10, it. 2 of the Act of 18th of July 2002 on provision of services by electronic means, after expressing the consent, on the basis of art. 6, it. 1 point a) of the above regulation;
5. e) participation in loyalty program of ORSKA company after expressing a separate consent on the basis of art.6 it. 1 point a) of the above regulation.

3) Your personal data processed for the above purposes indicated in it. 2) point a) and b) above, will be stored during the effectiveness of the quality guarantee period or warranty period for the defects of the products by ORSKA, and processed for the purposes indicated in it. 1) point c)-e) – will be stored until the consent has been cancelled, no later however than until the completion of organizing commercial, marketing purposes by ORSKA company, including promotional campaigns or conducting loyalty program.

4) In case of necessity to deliver the product by ORSKA purchased by You, the personal data may be made available to the postal operators or carriers exclusively for the purposes of delivery of the order.

5) You possess the right of access to the text of your personal data and the right to correct it, remove, limit to process, right to transfer data, right to lodge an objection, right to withdraw the consent at any time without the influence on the consistency with the right to process, which was made on the basis of the consent before it has been withdrawn;

6) You have the right to lodge a complaint to GIODO (after 25th of May 2018 – to President of the Personal Data Protection Office – PUODO) when you state that processing of Your personal data infringes the provisions of the above regulation;

7) providing personal data is voluntary, however providing personal data indicated in it. 2) point a) and b) above is the condition to conclude and realize the agreement; the consequence of failing to provide it is the lack of sale of the products by ORSA to You;

8) Your data will be processed in an automated manner, including also in the form of profiling. Automated decision making will take place on the basis of the assessment of the personal factors, behavior, preferences as to the selection of the product of a defined type, price or purpose of the product, consequence of such processing will be automatic adjusting of the product offer without human intervention.
